Co-creating better life outcomes by putting humans back in human services.
technology • data collection • increased social capital
The story begins…
In 2018, a coalition of New Britain social service providers introduced NB CARES, a groundbreaking Passport for Success aimed at empowering residents. For the founders, this initiative marked a pivotal moment in reshaping how social services could be delivered.
The vision? To revolutionize social mobility by gamifying the system, blending quantitative and qualitative measures to track real impact on the lives of low-income residents. Utilizing mobile technology, the goal was to swiftly eliminate barriers, ensuring individuals stayed on their path to success — all while fostering social capital and connections within the community.
The Social Drivers of Health
-
Education
An education is not just about earning a diploma. It’s building a compass - then a map - to living your best life.
-
Employment
What do you like to do? What do you do well? And what pays you? Getting ahead is easier if you can answer these questions.
-
Financial Literacy
Getting ahead and not just getting by requires hard work. Getting money to work for you rather than working for money requires knowledge.
-
Healthcare
Your health is your wealth. Get a physical, go to a dentist, check your vision, see a therapist, and visit a nutritionist. You’ll live longer!
-
Housing
Finding affordable housing is tough. But the steps are easy to follow for those willing to be patient and put in the work.
It’s all about who you know.
While society emphasizes networking from a young age, many in low-income situations face a task-focused reality. Our innovative passport app redefines these tasks as steps toward progress, creating a clear roadmap. Each task becomes an achievement and forms a valuable dataset. It also cultivates a supportive community, offering guidance, answers, and opportunities, effectively building a village around each participant.
All new access to human services.
The Social Health Passport app revolutionizes social mobility. Gone are the days of disjointed appointments and isolated agencies. This app facilitates seamless collaboration and communication among agencies, acting like sheet music for coordinated efforts. For users seeking human services, the Social Health Passport becomes their version of a AAA card, where membership has its advantages.
Going beyond the expected.
Not only do we track and quantify progress via task completion, but we also keep track of “vital signs” and any barriers that our clients face — All of which are generated in a report for case managers to review.
